Seamless process
How It Works
01 Refer a patient
Send over the patient information via fax (616-841-9693) or use our online, HIPAA compliant digital referral form (found on our website).
02 We'll Schedule the patient!
Using the information you provided, we will contact the prospective patient to schedule their initial office visit and send them their estimated insurance benefits & cost prior to their scheduled appointment.
03 Start of Care
After the patient's first office visit, we will send you their initial visit notes to let you know diagnoses, treatment approach, duration of active care, and next steps.
04 Discharge of care
If anything important comes up during their active care, we will inform you. At the end of their treatment (6-12 visits maximum for most new patients), you will receive their last visit notes with discharge information and response to care.
